I and/or my firm acknowledge responsibility to pay all invoices received from Accu-Beta within 30 days of the invoice date. After 30 days, 1.5% per month will be levied on unpaid balance. Additionally, if the invoice goes beyond 60 days, I understand that I and/or my firm will be charged a late fee of $100.

In addition, a 33% collections fee will be assessed if a collection attorney is needed at any time. I also understand that it is my right to get an estimate of the cost before any services are rendered.
